We will meet with our patient(s) face to face for complete assessment of environmental and medical factors.  We maintain consistent communication with all family members involved in the care of each patient.  


Following our initial consultation, we will write an individualized care plan for the patient and all family involved.  This could vary from a simple meeting with the patient to several times a week to twice a month.  

  • Review of physical and cognitive (mental) status.

  • Assessment of nutritional needs.

  • Assessment of social functioning.

  • Identification of safety issues.

  • Safety Walk of present/future living conditions and making suggestions if necessary.

  • Review of overall quality of life with guidance in achieving the highest quality of care possible.

  • Based on the assessment, create a Plan of Care for the individual and caregiver to follow.

  • Monitoring and oversight of care in-home, nursing home,     assisted living, memory unit.

  • Recommending technologies to add to security or safety.

  • Observe changes and potential risks of exploitation or abuse.

  • Provide options and oversight for direct caregivers in your home.

  • Helping families evaluate and select the appropriate level of housing or residential options.​

  • Determine the types of services that are right for a client and assisting to engage and monitor.

  • Coordinating the care of a loved one for families that live at a distance, including crisis management.

  • Local, cost-effective resources are identified and engaged as needed.

  • Monitor individual’s adherence to medical orders and instructions.

Kara brings 20 years of experience as a Registered Occupational Therapist and was intimately involved as a Caregiver/Guardian after her own mother who suffered a Traumatic Brain Injury in 1997. Kara has successfully worked with entire Healthcare teams in Hospitals, Long Term Care Facilities, Assisted Living Facilities, and Home Care settings. Kara has also created and led a Caregiver Support group embracing all caregivers located in her community called "Saturdays at 2".
